However, neither the membership fees noted above, nor the services provided in the above-listed packages are covered by Medicare. In Accordance with Medicare regulations, Dr. Fozouni OB/GYN & Assciates DBA La Femme Health & Beauty requires all Medicare patients to complete a Medicare Advanced Beneficiary Notice (“ABN”) prior to receiving services as part of our Concierge Membership Program. The purpose of an ABN is to ensure you are aware that the payments made by and services you receive pursuant to this program are not covered by Medicare. If you decide to enroll in the Concierge Membership Program we will provide you with the required ABN and will be happy to answer any questions you may have.
